Spina Bifida
บทนำ
Spina Bifida เป็นความบกพร่องของกระดูกไขสันหลัง มีถุงยื่นผ่านจากกระดูกไขสันหลังออกมาตามตำแหน่งที่บกพร่องนั้น พบบ่อยที่สุดที่บริเวณ lumbosacrum (ร้อยละ 63) มี hydrocephalus ร่วมด้วยร้อยละ 80-90 อาจเป็นองค์ประกอบหนึ่งของ Arnold-Chiari type II กว่าร้อยละ 90 เป็น myelomeningocele คือ ส่วนที่ยื่นออกมามีทั้ง CSF และเนื้อไขสันหลัง
Spina bifida แบ่งออกเป็นสองชนิดคือ ชนิดเปิด (open) ถุงที่ยื่นออกมาคลุมด้วยเยื่อหุ้มใส ๆ พบได้ร้อยละ 80 และชนิดปิด (closed) พยาธิสภาพปกคลุมด้วยผิวหนังหรือเยื่อหุ้มหนา พบได้ร้อยละ 20
ลักษณะทางคลื่นเสียงความถี่สูง
1. อาการแสดงโดยตรง
- กระดูกสันหลังแยก
- Myelomeningocele sac ยื่นออกจากด้านหลังของกระดูกไขสันหลัง
2. อาการแสดงโดยอ้อม
- Ventricles โต พบร่วมด้วยกว่าร้อยละ 75
- Lemon sign เป็นลักษณะการยุบลงของกระโหลกคล้ายมะนาวถูกกด ซึ่งเกิดขึ้นที่กระดูก frontal พบได้กว่าร้อยละ 95 พบในช่วงอายุครรภ์ 24-34 สัปดาห์ แต่ในทารกปกติก็พบ lemon sign ได้ราวร้อยละ 1
- Cerebellar (banana) sign หรือการหายไปของ cisterna magna ซึ่งทารกปกติทุกคนจะต้องเห็น cisterna magna ถ้าตรวจไม่พบแสดงว่า posterior fossa ถูกกดเบียดจนทำให้ cerebellum ถูกดันแผ่แบนไปหุ้ม midbrain เห็น cerebellum เป็นรูปกล้วย ความถูกต้องในการทำนาย open spina bifida โดยอาการแสดงที่ศีรษะ
การวินิจฉัยแยกโรคของ Lemon sign ของกระโหลกศีรษะ
- พบบ่อย ศีรษะปกติ
- พบบ่อยรองลงไป Open neural tube defect (Arnold-Chiari type II)
- พบไม่บ่อย Cephalocele
- พบน้อย น้ำคร่ำน้อยมาก
- Thanatophoric dwarfism (cloverleaf skull)

3. ความผิดปกติอื่นที่พบร่วมด้วย
- ระบบประสาทและกล้ามเนื้อที่เป็นผลจากพยาธิสภาพโดยตรง เช่น clubfoot
- ความผิดปกติอื่นของระบบประสาทส่วนกลาง คือ hydrocephalus, ACC, DWM เป็นต้น
- ความผิดปกตินอกระบบประสาท อาจมีได้ถึงร้อยละ 50 ได้แก่ ความผิดปกติของหน้า ไต
- ทางเดินอาหาร ทรวงอก แขนขา เส้นเลือดแดงสายสะดือเส้นเดียว และความผิดปกติของโครโมโซมสูงขึ้น
