Osteogenesis Imperfecta Type II

บทนำ

Osteogenesis imperfecta (OI) เป็นกลุ่มความผิดปกติในการสร้างคอลลาเจน ส่วนใหญ่ที่วินิจฉัยได้ก่อนคลอดเป็นชนิด type II ซึ่งเป็นชนิดที่เลี้ยงไม่รอด ถ่ายทอดทางพันธุกรรมแบบ autosomal recessive แต่บ่อยครั้งที่ตรวจพบเกิดจากการกลายพันธุ์ใหม่ของยีน ที่วินิจฉัยได้ก่อนคลอดนั้นมีเพียงน้อยรายเท่านั้นที่มีประวัติในครอบครัว กระดูกเปราะ และหักหลายแห่งตั้งแต่อยู่ในครรภ์ แขนขาสั้น ทรวงอกสั้นแต่ไม่แคบ กระดูกซี่โครงเป็นตุ่ม ๆ แบบลูกปัด กระโหลกอ่อน โปร่งเสียง มี blue sclera น้ำหนักน้อย อาจตายคลอดหรือตายในระยะแรกคลอด

ลักษณะทางคลื่นเสียงความถี่สูง

  • แขนขาสั้นมาก (micromelia) โก่ง หักมุม หนาตัวขรุขระในหลายบริเวณซึ่งเกิดจากแคลลัสที่มีกระดูกหักบ่อย ทำให้ยากที่จะเห็นตลอดแนวของกระดูกท่อนยาวในระนาบเดียวกันได้
  • กระดูกมีความเข้มเสียงต่ำทั่ว ๆ ไป (hypomineralization) กระโหลกอาจโปร่งมากจนสามารถเห็นร่องซัลคัสและไจรายของสมองได้ชัดเจนกว่าปกติ และกระโหลกอ่อนจนสามารถใช้หัวตรวจกดยุบได้โดยผ่านทางหน้าท้องมารดา
  • มีกระดูกซึ่โครงหักหลาย ๆ บริเวณ ทำให้ทรวงอกแฟบ
