Exencephaly (Acrania)
บทนำ
Exencephaly เป็นภาวะที่กะโหลกศึรษะหายไปบางส่วนหรือทั้งหมดในทำนองเดียวกับภาวะ anencephaly (1-3) แต่ยังคงมีเนื้อสมองอยู่ ลอยอิสระในน้ำคร่ำ เชื่อว่าเป็นภาวะที่จะดำเนินไปสู่ anencephaly เมื่อเนื้อสมองถูกย่อยสลายไป พยากรณ์โรคไม่ดี ไม่สามารถมีชีวิตรอดได้
ลักษณะทางคลื่นเสียงความถี่สูง
- ไม่พบกะโหลกศีรษะ วัด BPD ไม่ได้ แต่เห็นเนื้อสมองในปริมาณมากลอยอิสระในน้ำคร่ำ
- วิวหน้าตรงของใบหน้าให้ลักษณะ frog face appearance
- รูปร่างและโครงสร้างภายในสมองผิดปกติ
- การตรวจในไตรมาสแรกอาจพบสมองปูดออกไปด้านข้างทั้งสอง เป็น bilobed head ซึ่งให้ลักษณะที่เรียกว่า Mickey Mouse head
- น้ำคร่ำขุ่น (ประมาณเกือบร้อยละ 90)
- ควรวินิจฉัยแยกโรคจากภาวะกะโหลกโปร่งบาง เช่น osteogenesis imperfecta type II, hypophosphatasia เป็นต้น
