Abnormal Limb Posture
บทนำ
มือเท้าอยู่ในท่าผิดปกติ : เช่น ข้อมืองอตรึงอยู่กับที่ตลอดเวลา อาจเกิดจากการหายไปของกระดูกเรเดียส เช่นกลุ่ม trisomy 18 หรือ TAR syndrome หรือ ความผิดปกติของกล้ามเนื้อ เช่น Pena-Shokier syndrome เป็นต้น การบิดเบี้ยวของมือเท้า (clubhand, clubfoot) ที่ไม่เกิดจากการกดทับในกรณีน้ำคร่ำน้อย มักจะเป็นส่วนหนึ่งของกลุ่มโรคใดโรคหนึ่ง ซึ่งต้องตรวจค้นหาอย่างละเอียด
เท้าปุก
ส่วนใหญ่เป็นชนิด ฝ่าเท้าหมุนเข้าด้านใน และเอาด้านข้างของเท้าพลิกลงล่าง (telepes equinovarus) สาเหตุที่พบได้บ่อยที่สุดคือ ภาวะน้ำคร่ำน้อย รองลงไปคือประวัติในครอบครัว นอกจากนั้นอาจสัมพันธ์กับกลุ่มอาการอื่น ๆ ได้แก่
ความผิดปกติของการสร้างกระดูก ระบบประสาท (เช่นใน spina bifida) และกล้ามเนื้อ
- arthrogryposis
- diastrophic dysplasia
- Pena-Shokeir phenotype
- Larsen’s syndrome
- Trisomy 18 (มี clubfoot ร้อยละ 30)
- Trisomy 13 (น้อยกว่า trisomy 18)
Clenched hand
นิ้วเหลื่อมกัน (overlapping fingers) หรือ clenched hand ภาพคลื่นเสียงจะพบว่านิ้วมือไม่เรียงกันสม่ำเสมอเหมือนปกติ และเป็นอยู่เช่นนั้นตลอดเวลา อาจเห็นนิ้วชี้กันไปคนละทางอยู่อย่างคงที่ พบได้บ่อยมากในกลุ่ม trisomy 18, 13 และอาจเป็นส่วนหนึ่งของกลุ่มอาการอื่นๆ เช่น Smith-Lemli-Opitz syndrome, Multiple pterygium syndrome, Freeman-Sheldon syndrome เป็นต้น
