ต่อมไร้ท่อเชียงใหม่ หัวใจคือวิชาการ พัฒนางานวิจัย ให้บริการมาตรฐานสากล

ปณิธาน
หน่วยต่อมไร้ท่อและเมแทบอลิซึม ภาควิชาอายุรศาสตร์ เน้นการฝึกอบรมแพทย์ประจำบ้านและแพทย์ประจำบ้านอนุสาขา ให้มีความรู้ความสามารถในระดับสากล ฝักใฝ่ในการฝึกอบรมตนเองอย่างต่อเนื่อง มีคุณธรรมและจิตสำนึกในการรับใช้สังคม ส่งเสริมงานวิจัยของผู้เข้ารับการฝึกอบรม ให้บริการชุมชนในการดูแลโรคต่อมไร้ท่อที่มีคุณภาพมาตรฐาน เหมาะกับทรัพยากร และดำเนินการพัฒนาระบบประกันคุณภาพให้เข้มแข็ง

พันธกิจของการฝึกอบรม/หลักสูตร
หน่วยวิชาต่อมไร้ท่อและเมแทบอลิซึม ภาควิชาอายุรศาสตร์ คณะแพทยศาสตร์ มหาวิทยาลัยเชียงใหม่ กำหนดพันธกิจด้านการศึกษาของหลักสูตร คือ

“ผลิตอายุรแพทย์โรคต่อมไร้ท่อและเมแทบอลิซึมที่มีความรู้ความสามารถตามมาตรฐานวิชาชีพ
และมีคุณธรรมจริยธรรม เพื่อตอบสนองความต้องการของชาติ”

โดยมุ่งหมายให้ผู้รับการฝึกอบรม
1. มีความรู้ความชำนาญด้านโรคต่อมไร้ท่อและเมแทบอลิซึมตามมาตรฐานวิชาชีพสามารถปฏิบัติงานได้ด้วยตนเองโดยไม่ต้องมีการกำกับดูแล
2. มีความสามารถในการเรียนรู้เพื่อพัฒนาวิชาชีพอย่างต่อเนื่อง และสามารถทำงานวิจัยทางการแพทย์ได้อย่างถูกต้องตามหลักวิชาการและหลักจริยธรรมการวิจัยทางการแพทย์
3. มีคุณธรรมจริยธรรม ทัศนคติเจตคติที่ดีและมีพฤติกรรมที่เหมาะสมต่อเพื่อนร่วมงานทั้งในวิชาชีพของตนเองและวิชาชีพอื่นๆ รวมทั้งผู้ป่วยและญาติมีความเอื้ออาทรและใส่ใจในความปลอดภัยโดยยึดถือผู้ป่วยเป็นศูนย์กลางมีจิตสำนึกในการรับใช้สังคม
4. มีความเป็นมืออาชีพในการดูแลผู้ป่วยแบบองค์รวม ตั้งแต่การสร้างเสริมสุขภาพการป้องกันโรค การตรวจวินิจฉัย และการรักษา
5. มีทักษะในการสื่อสารและปฏิสัมพันธ์สามารถทำงานเป็นทีมร่วมกับบุคลากรอื่นรวมทั้งการปฏิบัติงานแบบสหวิชาชีพได้ รวมถึงมีความสามารถในการถ่ายทอดความรู้ให้แก่นักศึกษาแพทย์ ทีมแพทย์ผู้เชี่ยวชาญด้านอื่นหรือผู้ร่วมงาน

มีความรู้ความเข้าใจในระบบสาธารณสุขของประเทศและมีความสามารถเกี่ยวกับการบริหารจัดการ เพื่อที่จะสามารถให้บริการสุขภาพแก่ชุมชนอย่างมีคุณภาพตามมาตรฐาน โดยเฉพาะอย่างยิ่งการดูแลปัญหาที่พบบ่อยของผู้ป่วยทางภาคเหนือของประเทศไทย

การให้บริการ
1. แผนกผู้ป่วยนอก
1.1 ทำการตรวจและรักษาผู้ป่วยที่ห้องตรวจเบอร์ 21 สัปดาห์ละ 3 วัน ตั้งแต่เวลา 8.00-12.00 น. ดังนี้
– วันจันทร์คลินิกโรคเบาหวาน
– วันพุธ และวันศุกร์ คลินิกโรคต่อมไทรอยด์ ต่อมหมวกไต, ต่อมใต้สมองฯลฯ รวมทั้งโรคที่เกี่ยวกับเมแทบอลิซึม
1.2 ให้คำแนะนำเกี่ยวกับการปฏิบัติตนในผู้ป่วยเบาหวาน เช่น การควบคุมอาหาร, การออกกำลังกาย, การรับประทานยา, การฉีดยา, การดูแลเท้า, การตรวจน้ำตาลในเลือดปลายนิ้วด้วยตนเอง เป็นต้น
1.3 ตรวจเท้าผู้ป่วยเบาหวาน ให้การดูแลในผู้ป่วยที่มีปัญหาเรื่องเท้าและให้คำแนะนำการดูแลเท้า และการเลือกรองเท้าที่เหมาะ
1.4 นอกจากนี้ยังให้บริการต่อไปนี้
– ตรวจต่อมไทรอยด์ด้วยเครื่องความถี่เสียงสูง (Ultrasonography)
– ทำ Fine needle aspiration ต่อมไทรอยด์เพื่อการวินิจฉัยโรค
– ทำ Percutaneous ethanol injection ในผู้ป่วยที่เป็นถุงน้ำที่ต่อมไทรอยด์
– ให้คำแนะนำเกี่ยวกับขั้นตอนการรักษาและการปฏิบัติตนในผู้ป่วยโรคคอพอกเป็นพิษ, โรคมะเร็งต่อมไทรอยด์, โรค Adrenal insufficiency โรคอ้วนและโรคไขมันในเลือดสูง
2. แผนกผู้ป่วยใน
2.1 รับปรึกษาและดูแลผู้ป่วยในหอผู้ป่วยอายุรกรรมที่มีปัญหาโรคระบบต่อมไร้ท่อและเมแทบอลิซึม
2.2 รับปรึกษาจากต่างแผนกในปัญหาของผู้ป่วยโรคระบบต่อมไร้ท่อและเมแทบอลิซึม
2.3 ให้ความรู้เกี่ยวกับการปฏิบัติตนในโรคเบาหวาน, โรคไขมันในเลือดสูง, โรค Adrenal insufficiency แก่ผู้ป่วยและญาติที่มานอนรับการรักษาในโรงพยาบาลทั้งในแผนกอายุรกรรมและต่างแผนก
3. ทำการทดสอบผู้ป่วยทางห้องปฏิบัติการเพื่อการวินิจฉัยโรค (Dynamic endocrine tests) เช่น
– Oral glucose tolerance test (OGTT)
– Insulin induced hypoglycemia
– Growth hormone suppression test
– Water Deprivation test
– ACTH stimulation test
– Saline loading test
4. บริการตรวจเลือดทางห้องปฏิบัติการดังนี้
– Serum Insulin
– Plasma aldosterone
– Plasma renin activity
– Anti TSHR
– Plasma ACTH
– Urine metanephrine , Nor-metanephrine

บริการวิชาการแก่ชุมชน
1. รับเชิญบรรยายให้ความรู้แก่สมาคมและหน่วยงานต่างๆ เกี่ยวกับโรคระบบต่อมไร้ท่อและเมแทบอลิซึม
2. จัดอบรมผู้ป่วยเบาหวานและญาติ ในโครงการ “ค่ายผู้ป่วยเบาหวานสัญจร” ปีละ 1 ครั้ง
3. จัดกิจกรรมในวันเบาหวานโลก ทุกวันที่ 14 พฤศจิกายน ของทุกปี

• เกณฑ์หลักสูตรการฝึกอบรมแพทย์ประจำบ้าน อนุสาขาอายุรศาสตร์โรคต่อมไร้ท่อและเมแทบอลิซึม

1.2. คู่มือการฝึกอบรม

• คู่มือฝึกอบรมแพทย์ประจำบ้าน อนุสาขาอายุรศาสตร์โรคต่อมไร้ท่อและเมแทบอลิซึม

1.3. เกณฑ์การคัดเลือกแพทย์ประจำบ้านต่อยอด
ผู้เข้ารับการฝึกอบรมจะต้องเป็นผู้ที่ได้รับใบอนุญาตประกอบวิชาชีพเวชกรรมแล้ว และต้องมีคุณสมบัติอย่างใดอย่างหนึ่งดังต่อไปนี้
1.1 เป็นผู้ได้รับหนังสืออนุมัติหรือวุฒิบัตรฯ สาขาอายุรศาสตร์
1.2 เป็นแพทย์ประจำบ้านปีสุดท้ายในการฝึกอบรมเพื่อวุฒิบัตรฯ สาขาอายุรศาสตร์
1.3 เป็นผู้มีสิทธิสอบเพื่อหนังสืออนุมัติหรือวุฒิบัตรฯ สาขาอายุรศาสตร์ในปีการศึกษานั้น

2. รับสมัครแพทย์ประจำบ้านและแพทย์ประจำบ้านต่อยอด

• รับสมัครแพทย์ประจำบ้านอนุสาขาอายุรศาสตร์ โรคต่อมไร้ท่อและเมแทบอลิซึม ปีการศึกษา 2567 (New)

3. ทำเนียบแพทย์ประจำบ้านต่อยอด
