Treatment outcomes and factors associated with mortality rate among pediatric patients admitted to pediatric intensive care unit at Chiang Mai University Hospital.

Abstract

Background: Mortality in pediatric intensive care unit (PICU) can be associated with diseases which patients suffered, demographic profiles and underlying diseases, complications and nosocomial infections. And there is no clinical data in PICU of Chiang Mai university hospital which studied before.

Objectives: We aim to determine demographic profiles, treatment outcomes and factors associated to mortality rate among pediatric patients admitted to pediatric intensive care unit at Chiang Mai University hospital.

Method: A retrospective cohort study among pediatric patients admitted to PICU 1 of Chiang Mai university hospital between 1 January 2014 - 31 December 2014. We collected data on demographic profiles; underlying diseases; source and diagnosis at admission; laboratory features; nosocomial infections; treatment in PICU; length of stay; mortality rate and treatment outcomes. The factors which associated mortality were modeled using multiple logistic regression method.

Results: A total of 177 admissions admitted to PICU 1 (54.2% males). Mean age was 49.55 months. Most of them came from general pediatric ward (42.9%). Diagnoses included postoperative cares (47.5%), the rests were non-postoperative patients as respiratory problems were the majority (23.2%). Most common underlying disease was from respiratory system (32.8%). Mean LOS was 8.4 days. PICU mortality rate was 12.4%. The most common cause of death was septic shock/septicemia/multiorgan failure. Significant mortality risk factors were females, non-postoperative patients, vasoactive drugs used and platelet transfusion.

Conclusion: Our PICU mortality rate was 12.3%. The significant mortality risk factors were females, non-postoperative patients, VAP infections, vasoactive drugs used and platelet transfusion. These results can be used to improve mortality rate in our setting in the future. (Thai J Pediatr 2016 ; 55 : 149-160)
