Peritoneal dialysis catheter survival time in pediatric patients at Chiang Mai university hospital

Abstract

Background: Peritoneal dialysis (PD) is considered the mainstay for renal replacement therapy in children when dialysis is required. This study aimed to evaluate catheter longevity and factors associated with PD catheter duration.

 

Methods: Medical records of children who started peritoneal dialysis before the age of 18 years, from 7 December 2004 to 20 November 2019 at Chiang Mai university hospital, were retrospectively reviewed. Catheter survival was analyzed using Kaplan Meier method, factor associated with catheter survival was analyzed using cox regression.

 

Results: Thirty-six patients were identified; 22 boys (61%) and 14 girls (38%), the median age at initiating PD was 12 years (IQR 8-14). The most common cause of ESRD were unclassified glomerulonephritis (n=8, 22%). The incidences of complications were as follows: infected CAPD were observed in 11 (30.6%) of 36 patients, 7 (19.4%) patients had PD malfunction. The median duration of PD was 24 (IQR 9-41) months. Catheter survival rate was 75% at 12 months, 45% at 24 months, 25% at 36 months. Patients who start PD at age < 2 years old were statistic significantly shorter PD duration. Using Cox-regression analysis, age <2 years was a risk factor predicting the shorter duration of PD (hazard ratio (HR) 21, 95% CI, 0-1.822, p = 0.8). However, though not statistically significant

 

Conclusion: The results of our study demonstrated that age may be an important factor for the duration of PD. Further randomized prospective or retrospective studies are necessary to determine a factor for PD catheter survival in pediatric patient.
